Client Portal

The client portal will allow you to have access to:

•Billing documents such as invoices, statements, and superbills

•Make any payments due 

•Make an appointment and view upcoming appointments with your therapist 

•View all practice policies and consents that were signed 

•Sign any new forms sent to you by the practice 

Simply visit Evaluations For Change main client portal page. You will be prompted to enter the e-mail address associated with your account (this will be the same e-mail address you provided during your intake) and the password you created. 

If you don't receive your telehealth reminder/link please check your spam/junk folder.
